Forms:

New clients will need to fill out a health information form.  Upon booking an appointment, a valid email address will be requested and a form will be emailed to you via the website MassageBook.  Please complete the form prior to your appointment.

If you do not have access to email, please let me know.  Arrangements can be made for you to fill out your form when you arrive for your appointment.  Please arrive 15-20 minutes before your appointment to complete your health form.

Health and Contraindications:

The client's health and wellbeing are of utmost concern.  Clients should be aware that massage therapy may be inadvisable or require doctor's permission with certain medical conditions.  Some conditions that are localized may require the therapist to avoid the affected area.  

Massage cannot be performed if you are currently experiencing any of the following:

Fever

Cold/Flu

Any contagious airborne bacterial, viral, or fungal infection

Acute rheumatoid arthritis

Intoxication

Untreated circulatory problems
